To highlight our history in working with the homeless, we have 4 programs that deal with patients with housing issues:

PATH is an outreach program serving the homeless population. They provide permanent housing, and case management services. They link clients to services, provide housing options for the homeless, and assist individuals on the street with personal care items (food, blankets, scarves, gloves, etc.).

The Supportive Housing Program has fourteen subsidized housing units granted by the Division of Mental Health. SHP workers provide referral services and case management services, in addition to assisting the consumer with independent living skills and provide subsidized rent through the grant.

HUD is a transitional housing program serving the homeless population. HUD has five apartments in the community where a consumer lives and pays rent according to their income. The workers provide case management services, linking consumers to appropriate services.

Shelter Plus Care has twelve subsides housing provided by the HUD grant. The workers provide case management services as well as assisting the client with independent living skills.

Existing Building

The existing building is a three story structure that is attached to a one story building. There are approximately eighteen rooms of various sizes on each floor. The basement of the building is unoccupied and used for storage. There is a second exit in the rear of the building.

Proposed Building Design

The building will be renovated to include approximately 24 efficiency apartments with each floor containing 8 apartments. The building does not currently contain an elevator, but will house one upon completion of the project. Each apartment will have with a kitchenette with a stove and refrigerator, as well as a private bathroom. Each floor will also have common spaces that will be used as a library, game room, and television/music room. The first floor will also include a public restroom for visitors. There will be a laundry room with coin operated washers and dryers. Two handicap accessible apartments are also planned for the first floor.
