The Projects for Assistance in Transition from Homelessness (PATH) program reaches some of the most vulnerable members of our community. It delivers essential services, resources to improve the mental health and well being of people who are homeless. Services include:

  • Outreach services
  • Screening and diagnostic services
  • Habilitation and rehabilitation services
  • Community mental health services
  • Alcohol or drug treatment services (for people with mental illnesses and co-occurring substance use disorders)
  • Case management services
  • Supervisory services in residential settings
  • A limited set of housing services and services to help clients access housing resources

All patients are treated by a multi-disciplinary staff consisting of child and adolescent psychiatrists, therapists, counselors, social workers and nurses who provide consistent compassionate care.

Hours of Operations:

Located in the East Pavilion the PATH Program operates Monday through Friday 8:00am to 6:00pm.
